Summary

Issue: On certain vehicles, the rear view camera system does not return to the default setting when the transmission is shifted to reverse. This could cause a reduction in rear visibility if a driver had previously adjusted the camera display. Canadian regulations require that the rear view image returns to its original setting each time the camera is turned on. Safety Risk: Reduced rear visibility could increase the risk of a crash. Corrective Actions: The company will notify owners by mail and instruct them to take their vehicle to a dealer to update the software for the rear view camera.

Summary

On certain vehicles equipped with a mechanical key ignition system, a spring in the ignition switch could wear and break. This may allow the ignition key to inadvertently move from the ON position to the accessory (ACC) position while driving, causing the engine to shut off and the airbag system to lose power. The resulting loss of motive power, and change in steering and brake forces could increase the risk of a crash causing injury and/or damage to property. Additionally, in the event of a crash, this may also cause the airbags not to function. Correction: Dealers will inspect the lot number of the ignition switch, and replace it with a new one if it is affected. Note: Until the correction is performed, owners are advised to remove all objects from the ignition key ring, such as additional keys, key chains, etc.

Summary

On certain vehicles, the inflator diffuser installed in certain side curtain air bag modules may not have been manufactured correctly. If the vehicle were involved in a crash that warranted side curtain airbag deployment, this could potentially lead to the diffuser detaching. If the diffuser were to detach, it could become a projectile in the occupant compartment, which could result in the side curtain airbag to not fully inflate. Both of these conditions could increase the risk of injury in a crash. Correction: Dealers will replace the side curtain air bag modules.

Summary

On certain vehicles, the ring plate that secures the frontal passenger airbag cushion to the assembly housing may not have sufficient yield strength due to a manufacturing defect. This could cause the airbag cushion to separate from the ring plate when the airbag is deployed which could increase the risk of injury. Correction: Dealers will replace the front passenger side airbag module.

Summary

On certain vehicles, the driver's foot could contact the edge of the center console lower trim panel, which could impede smooth accelerator pedal operation. The panel edge could also catch the driver's shoe, potentially resulting in a delay in the driver being able to shift their foot from the accelerator to the brake pedal. This could increase stopping distances, potentially resulting in a crash causing injury and/or damage to property. Correction: Dealers will modify the lower trim panel.

Summary

Certain vehicles fail to conform to Canada Motor Vehicle Safety Standard 206 – Door Lock and Door Retention Components. A manufacturing error could cause the left front and/or left rear door latches to fail to fully latch and fail to meet the requirements of the standard. As a result, occupants may be at greater risk of injury in a crash, and unbelted occupants could fall from the vehicle and sustain injury. Correction: Dealers will inspect latches and replace as necessary.

As a result, the seat anchorage could fail to meet the strength requirements of the standard. This could increase the risk of injury to rear seat occupants in a crash.
